The Board will provide verbal verification of a doctor's certification status by calling (312) 202-5900. The Board will only confirm the status of the doctor's certification and no other information will be released.
If you need additional information pertaining to a doctor's certification, the Board will provide a written verification for a pre-paid fee of $50.00. All requests should be made in writing and sent to the Board office.
